Under the General Data Protection Regulation (GDPR), you have the right to access, rectify, erase, restrict, or port your personal data. Use this form to submit a data subject request to The Origin Institute® regarding data processed through the Sensorama™ platform.

    Response timeline: We will acknowledge your request within 5 business days and provide a substantive response within 30 days (GDPR Art. 12(3)). If your request is complex, we may extend this by up to 60 additional days and will inform you of the extension and the reasons for it.
  

Before You Submit

Are you a panelist?

If your data was collected during a sensory evaluation study, the organisation that invited you to participate is the data controller for your data. Please contact them first. If you cannot reach them, or if your request specifically relates to how Sensorama processes your data, you may use this form.

Are you a Sensorama customer or website visitor?

You may use this form directly. The Origin Institute is the data controller for your account data, contact form submissions, and website usage data.

Identity Verification

To protect your privacy, we need to verify your identity before processing your request (GDPR Art. 12(6)). We may contact you to request additional verification, such as:

Confirmation from the email address associated with your account.
Answers to security questions related to your account.
A copy of a government-issued ID (only in cases where other verification methods are insufficient, and only the minimum information necessary).

Request submitted successfully. We have received your data subject request and will acknowledge it within 5 business days. A confirmation has been sent to your email address. Please check your spam folder if you do not receive it.

What Happens Next

Acknowledgement (within 5 business days): We will confirm that we have received your request and inform you of any additional information or verification needed.
Verification: We may contact you to verify your identity. This is to protect your privacy and ensure that personal data is not disclosed to unauthorised persons.
Processing: Once verified, we will process your request. The specific actions depend on the type of request:
- Access: We will compile and provide a copy of your personal data in a commonly used electronic format.
- Rectification: We will correct the inaccurate data and confirm the changes.
- Erasure: We will delete your personal data, unless we have a lawful basis for continued retention (e.g., legal obligations).
- Portability: We will provide your data in CSV or JSON format.
- Restriction: We will restrict processing and inform you before any restriction is lifted.
- Objection: We will cease processing unless we demonstrate compelling legitimate grounds.
Response (within 30 days): We will provide a substantive response within 30 days of your request. If the request is complex, we may extend this by up to 60 days (we will inform you of any extension).

Fees

There is no charge for exercising your data subject rights. However, if requests are manifestly unfounded or excessive (for example, repetitive requests), we may charge a reasonable fee based on the administrative costs of providing the information (GDPR Art. 12(5)(a)) or refuse to act on the request.
